The Flexible Spending Account (FSA) Plan allows clergy and lay employees to reduce their out-of-pocket costs for medical expenses and dependent-care. Based on the amount an employee elects to put into the account each year, the employer deducts an amount from each paycheck and deposits it in the FSA. The amounts deducted are not subject to Federal Income tax or FICA tax. Click here for more information on what an employer needs to do to participate. A participant uses the following forms to manage his/her Flexible Spending Account:

Change in Status Form
Revocation of Benefit Election
