The UCC Health Plan provides optional vision coverage among its Plan offerings. Vision benefits are available through VSP, a nationwide network of vision care providers. For a summary of the benefits available, click here.
ONLINE: Open enrollment for the UCC Vision Benefits Plan is now available through March 15, 2022. Coverage is effective at 12:01 a.m. on April 1, 2022.

If you are a Pension Boards member but have never enrolled in the vision plan, click here to log in, enroll in vision and pay your vision premium.
If you have never been a Pension Boards Member and would like to enroll in the Vision Benefit Plan, you will need to complete the Vision Enrollment form. Click here to access that form.
If your employer is paying for your vision benefit, they will receive an email notification of your enrollment and must acknowledge your election via the Pension Boards’ employer portal. Please check the status of your enrollment by logging into the Pension Boards website and select “Vision Enrollment” on the “Benefits Selection” page to make sure your employer has approved your election. If your application still says “Pending,” please contact your employer.
Participants who wish to pay for their coverage on a self-pay basis, can elect to do so by submitting payment online.
The Vision Plan is offered on a stand-alone basis, regardless of participation in the UCC (Non-Medicare) Health Plan or UCC Medicare Advantage Plan. The Plan offers access to a nationwide network of eye care providers through VSP. Plan benefits include annual eye exams, prescription glasses or contact lenses, as well as discounts on lens options, additional glasses, and sunglasses.
Eyeconic is a special program for VSP plan participants that allows you to browse a wide selection of popular frames. You can try them on (virtually) and purchase online from the comfort of your own home. Participants also have the option to use their benefit at select Costco or Walmart locations.
